Ask Nurse Lynn: Anesthesia and Steroids

Question:

Female, 8 years old, deletion subtype

I thought during one of the seminars during the 2025 pws conference, that it was said that anytime someone with PWS gets put under with anesthesia that steroids should be automatically given. If this is true, I was wondering why, If there is a steroid that is preferred and what evidence I can show my provider? If it’s not true is there anything else besides anesthesia sensitivity and sleep apnea I should be letting my provider know about? My daughter 8 with deletion is going under for an endoscopy and biopsy, and has mild sleep apnea.

Nurse Lynn’s Response:

Some people with PWS may have trouble making enough stress hormones during surgery or illness. This is called central adrenal insufficiency (CAI). It does not happen to everyone with PWS, so doctors do not automatically give steroids to all children with PWS during anesthesia. But PWS experts say that doctors should think about giving stress-dose steroids if a child has signs of not handling stress well, like very low energy during sickness, low blood pressure, or low blood sugar. If steroids are needed, the one most often used is hydrocortisone. Even though steroids are not always required, it is very important for the anesthesia team to follow best practices for PWS. These include watching breathing closely because of sleep apnea and weak muscles, using the lowest safe amount of sedation, helping keep the child warm, making sure the stomach is empty to reduce the chance of choking, and giving extra monitoring after the procedure. Letting the team know about CAI and these PWS needs helps keep your child safe during anesthesia.
