Ask Nurse Lynn: Medical Causes for Weight Gain

Question:

Female, 18 years old, deletion subtype

My daughter is 18 years old & her weight over the past 6 months her weight has been increasing. We try to control her calories to 1000 per day. Her weight today was 115.7kg. She does 1hr exercise per day, ie. treadmill, swimming or 4 mile walk. but we are struggling. We have even tried Mounjaro injections which don’t seem to be helping. Any advice would be appreciated.

First, I just want to say that you are working incredibly hard. Limiting calories to around 1000 per day, making sure she exercises for an hour daily, trying treadmill, swimming, long walks, and even trialing medication like Mounjaro, that is not a lack of effort. Parenting and supporting someone with PWS through weight challenges can be exhausting.

Weight gain in an 18-year-old with PWS can happen even when families are doing “everything right.” Because PWS affects metabolism, muscle mass, hormones, and hunger regulation, sometimes we need to step back and look at the bigger medical picture.

Here are a few things you may want to discuss with her physician:

Review GH continuation with endocrinology if she is not currently on GH. Adults with PWS often benefit from continued GH for Lean muscle mass, fat metabolism and body composition. https://pmc.ncbi.nlm.nih.gov/articles/PMC7869190/

Check thyroid (including free T4)- even a mild case of hypothyroidism can lead to weight gain

Is a sleep study needed? Many individuals with PWS have disordered sleep which can promote weight gain and increase insulin resistance https://pubmed.ncbi.nlm.nih.gov/35172921/

You may want to try mixing up her workouts a bit with some resistance training with bands or some weight training with supervision.

This is always a delicate question, but it is worth gently reviewing whether there could be access to food outside structured supervision times. As hard as we try, none of us can be everywhere at once. Even small additional intake can offset a tightly controlled calorie plan in PWS.
