Ask Nurse Lynn: Deconditioning After Hospital Stay

Question:

Male, 4.5 years old, deletion subtype

Looking for insight into why my 4.5 year old son is falling more and who I should speak to because his endocrinologist says it’s out of his realm. He was in the hospital for 9 days with an infection in his throat that walled itself off and bad strep. He got out and now I guess he’s been falling more at school. Another mother told me this happened to her son and her son ended up paralyzed after a throat infection. I know it could be anything else, like even his hip contracture or the fact that he may need glasses or even just his hypotonia getting worse as we lowered the dose of his GH 6 months ago.

Nurse Lynn’s Response:

Being in the hospital for nine days with a serious throat infection and strep can make a child weak and tired. This is called “deconditioning,” and it can happen when kids are less active for a while. That weakness can lead to more falls. It is helpful to start by tracking when the falls happen at school and at home, including the dates, times, what he was doing, and whether he seemed tired or unsteady. This information can help doctors understand what is going on.

It would be reasonable to ask for a referral to a pediatric neurologist, who can check his muscle strength, balance, and reflexes. Since he has a hip contracture, following up with an orthopedic doctor to check his hip and walking pattern is also important. Having his eyes checked can help rule out vision problems that may cause tripping. Because his growth hormone dose was lowered, this may also affect his muscle strength and energy, so sharing these concerns again with his endocrinologist after other evaluations may be helpful.
