Transitions are changes from one activity to another, from one relationship to another, or from one setting to another.  In this booklet, we address the transition from school to adult services for students with disabilities, with special attention to young adults with Prader-Willi syndrome.  For the most part, we focus on the transitions of young adults who are receiving educational services in a special education setting, whether that setting is a special school, special education class, or mainstreamed class.

This booklet helps you understand what transition is, who provides services, and what you as a parent will need to do.  After Chapter 1 presents a basic overview, in separate chapters we discuss work (Chapter 2) and residential (Chapter 3) issues.  In Chapter 4, we describe how the transition from child to adult services is complicated when the young adult has Prader-Willi syndrome.  We end this booklet by discussing some remaining issues and problems.  The booklet also features several appendices about contracts and practices in each of the 50 states, as well as a glossary of terms (terms appearing in the glossary are noted in bold type throughout this booklet)
